The New Champion of PPARα Modulators: How Pemafibrate Is Changing the Lipid-Lowering Landscape?
Pemafibrate is a novel, highly selective peroxisome proliferator-activated receptor α (PPARα) modulator developed by Kowa Company of Japan.

This innovative drug significantly improves blood lipid metabolism by binding to PPARα and regulating the expression of genes involved in lipid metabolism. It can effectively reduce plasma triglyceride (TG) levels (by up to 50%), while increasing high-density lipoprotein cholesterol (HDL-C) levels and exerting positive effects on other lipid parameters.
Compared with existing drugs, pemafibrate has higher selectivity for PPARα, which may lead to better efficacy and safety. In addition to being mainly used to treat hyperlipidemia, pemafibrate also shows potential in indications such as non-alcoholic fatty liver disease, hypercholesterolemia, and primary biliary cholangitis.
Mechanism of Action and Clinical Efficacy
Pemafibrate is 2,500 times more selective for PPARα than fenofibrate, and its TG-lowering effect is not inferior to that of fenofibrate. Compared with currently used omega-3 fatty acids (which can lower TG by about 20%), pemafibrate can reduce TG levels by up to 50%. Multiple clinical studies have confirmed the significant effect of pemafibrate in lowering fasting serum TG and increasing HDL-C, as well as its sustained long-term action.

Structural formula of pemafibrate
The results of the PROMINENT trial published in the New England Journal of Medicine in 2022 showed that among patients with type 2 diabetes, mild-to-moderate TG elevation (200–499 mg/dL), low HDL-C (≤40 mg/dL), and receiving statin therapy, pemafibrate reduced median TG levels by 26.2% compared with placebo and produced beneficial effects on other lipid parameters.
Marketing and Development Status
Pemafibrate tablets (trade name: Parmodia) were approved for marketing in Japan in 2017 for the treatment of hyperlipidemia (including familial hyperlipidemia). In only three years, its market share surpassed that of fenofibrate. At present, important progress has also been made in the development of pemafibrate in China:
1. On October 19, 2023, Kowa Company submitted a marketing application for pemafibrate tablets as a Class 5.1 new drug to the Center for Drug Evaluation (CDE) of China's National Medical Products Administration, and the application has been accepted.

2. A 12-week, multicenter, placebo- and active-controlled, randomized, double-blind Phase 3 study has been completed in China to evaluate the efficacy and safety of pemafibrate in Chinese patients with high TG and low HDL-C.

3. In addition to hyperlipidemia, pemafibrate is being explored for other indications. Currently, indications for non-alcoholic fatty liver disease, dyslipidemia, and hypercholesterolemia have entered the clinical stage, and the indication for primary biliary cholangitis has advanced to Phase 2.
Market Prospects
Pemafibrate has performed impressively since its launch, with rapid market growth. In 2020, its market sales already surpassed those of fenofibrate, reaching USD 65.1 million, demonstrating good market potential.

Comparison of pemafibrate and fenofibrate sales data in Japan
As a novel PPARα modulator, pemafibrate shows significant clinical efficacy and great market potential in lowering blood lipids. With the completion of clinical studies in China and the advancement of its marketing application, pemafibrate is expected to provide Chinese patients with a new treatment option for hyperlipidemia.
Quality Control and R&D Support
Against this positive market outlook and clinical application background, the importance of drug development and quality control is increasingly prominent. To support the further development and quality assurance of pemafibrate, CATO provides pemafibrate impurity reference standards to help fully realize the potential of pemafibrate in clinical application and market expansion, while providing strong assurance for patient medication safety.
